The opportunity to receive a full-fledged education is of concern to both students of medical universities and those who are just preparing to enter medical school. And therefore, one question is extremely relevant for future doctors: will there be an internship in 2016-2017 or not?

Internship in 2017

According to state educational standards, a graduate of a higher educational institution with a medical profile must be fully prepared to work in the healthcare sector. Therefore, such a form of education as internship is completely abolished.

So, everyone who entered universities from 2013 and later, asking the question “will internships be canceled in 2017?”, should know:

  • On September 1, 2016, internships for pharmaceutical and dental specialties are canceled;
  • On September 1, 2017, internships for pediatric and therapeutic specialties are canceled.

Main changes

What will change with the cancellation of internships in 2017? What should future graduates expect? About all the most significant changes we will try to tell you in more detail.

The most significant innovation, which, in fact, forms the basis for the abolition of internships, is changes in the format of education. Practical development of the acquired skills will be carried out at the institute. By changing the educational process and integrating parallel practical experience into it, the ministry hopes to obtain active, work-ready specialists immediately after graduation.

In addition, it is planned to introduce a narrower specialization for “treating” specialties. In the sixth year of study, the second half of the year will be entirely devoted to the chosen “narrow” specialization, with simultaneous practice in medical institutions attached to universities.

But in order to work as a surgeon, transplantologist, cardiologist, neonatologist, and so on, the graduate will be required to complete residency. Two years of practical training under the supervision of an experienced practicing mentor for such specialties remains mandatory. That is, only internships are canceled in 2016-2017, but residency as a form of training is still relevant.

A graduate of a medical university will be able to begin practical work as a therapist or pediatrician immediately after receiving his diploma.

If previously, in order to obtain the right to work even as a local doctor in a clinic, a certificate of completion of an internship was required, then starting next year, university degrees will be sufficient.

In addition, in 2017, certification will be replaced by accreditation. In turn, accreditation for graduates is planned to be combined with state exams. Thus, according to experts, the quality of education should improve significantly.

To many, the cancellation of internships in 2017 seems like a very dubious innovation. After all, the usual form of education made it possible to acquire the necessary knowledge in practice, while sharing responsibility with already established specialists. However, according to the new educational standards, this same “practical training” will be included in the main course of training for future doctors. Thus, the fifth and sixth years of study, and for dentists and pharmacists the fourth and fifth years, will be entirely devoted to the acquisition of practical skills. To achieve this, future doctors will practice their acquired skills under the guidance of experienced specialists without interruption from the main course of study.

Why are transformations needed?

The main goal pursued by the abolition of internships is the solution personnel issue. It's no secret that in our time the shortage of doctors at the primary level of the healthcare system has become a real disaster. And the abandonment of post-institute one-year training should help in this matter.

The Ministry of Health has decided to go even further and “fix” a practical solution to the problem of the shortage of primary care physicians. Now all graduates who entered the so-called target sets, that is, according to quotas from regional ministry or medical institution, will have to fulfill the opportunity given to them. They will be assigned to regional clinics in the area from which the recruitment was made. And the young specialist will be required to work in the specified place for three years. In this way, the ministry hopes to fill the “burning” vacancies in first-level medical institutions.

By the way, in these same medical institutions, from whom future doctors were sent to study, students will also undergo practical training. That is, internship is integrated into the educational process, thereby increasing general level preparation.

However, teachers and experienced doctors express some doubt about the correctness of such a step. Will students have time to learn the material that is necessary to obtain the proper skills? Only time can answer this question.

The opportunity to receive a full-fledged education is of concern to both students of medical universities and those who are just preparing to enter medical school. And therefore, one question is extremely relevant for future doctors: will there be an internship in 2016-2017 or not?

The scheme of education in medical universities in Russia has changed radically. Due to the cancellation of internships, many are afraid that dropouts will come to clinics. The introduction of new medical standards divided doctors into two classes.

Since 2016, future local pediatricians and therapists begin studying under a shortened program, which involves six years of training. Potential surgeons, cardiologists and other medical specialists will have to spend 8 to 11 years studying.

As it was

Previously, all doctors studied for seven to eight years. It took six years to study at medical school, and then either an internship for a year or a two-year residency. Moreover, in some specialties it was necessary to go through both of these steps. However, for many medical specialists, one internship or residency to choose from was enough.

An intern and a resident are considered something like students in a medical institution, since they are not on the staff and do not receive salaries. They are essentially doing...

By September 2017, medical schools will say goodbye to internships. Who will treat us from the flu in 2017?

This is, perhaps, the question that should be asked by those who fell ill this year or happily avoided infection.

The fact is that 2016 and 2017 are a time of great change for medical students, and therefore for all of us. From September of this year, internships for dentists and pharmacists are canceled, and from September 2017 – for pediatricians and therapists (Federal Law No. 323 of November 21, 2011 “On the fundamentals of protecting the health of citizens in the Russian Federation”).

Only if a young doctor dreams of becoming a surgeon or, say, a cardiologist, will he need to complete...

Graduates of higher medical educational institutions are required to undergo primary postgraduate training - residency. With its help, future doctors are able to develop independent skills and achieve a certain level of mastery in such a difficult task as providing emergency care. medical care to the population.

Residency 2017 includes not only testing in medical specialties, but also conducts a qualifying stage based on passing exams. Resident education is considered expensive, since the cost per year is comparable to paying in advance for 2-3 courses at a medical institution.

Reform education is underway by leaps and bounds and is being implemented not only in high school– new educational standards have reached the system of higher educational institutions. New changes this time affected medical educational institutions.

Starting from 2016, internships are canceled in Russian medical universities. This is due to the transition to a federal state educational standard(hereinafter referred to as GEF) of the third generation. But what does this mean for ordinary people? Experts in the field argue that this approach to training future doctors may have a negative impact on the level of training of healers in white coats.

Physicians' knowledge may decline

Scientists analyzed the level of knowledge of graduates of medical universities and noted that this moment, even before the coming changes, it is very weak. It turns out that now, having barely received a diploma, newly minted medical workers, instead of undergoing an internship, are required to begin working in medical institutions in full.

Experts also note that a graduate education may not be enough to work in clinics. Many people are concerned about whether yesterday's medical students will be able to provide quality care. That is why the new training standard provides for enhanced practical training of future doctors, including on special medical simulators. And after receiving a diploma, every doctor is simply obliged to undergo additional training every five years and expand their knowledge.

Nobody canceled residency

However, residency remains. That is, a student, upon graduating from university, works as a doctor for some time, gains practical experience, and then enters residency in his field. So far the figure is three years. Nothing is written in the documents, but this particular period has already been discussed verbally several times. This is exactly the amount of time a doctor will need to complete the “young fighter course” in primary care (whether it be an outpatient appointment or other work in a clinic).

Then, if the doctor has a desire, he will be able to enter residency. At the same time, they also decided to increase the deadlines for the latter. Depending on the specialty, postgraduate training will range from one to five years.

Personnel problem solved

As the members of the coordinating council say, cancellation of internship in 2016 will provide an almost complete solution to the problem regarding the shortage of personnel in the field of medicine. The third generation Federal State Educational Standard will completely change the nature of training medical students. From the theoretical segment, the emphasis will be shifted towards practice. Young specialists with their wealth of knowledge and a fresh point of view on many medical issues will pour into the country's clinics. In addition, health workers will be required to undergo accreditation as medical specialists.

New medical models

New teaching models are being implemented everywhere both in secondary schools and in higher education. educational institutions. As a result of the abolition of internships in 2016, the training scheme in medical institutions in Russia may completely change.

The updated educational standard requires the abolition of internships, depending on the number of important reasons, one of which is the lack of doctors at outpatient clinics in municipal clinics. The scientific plans of the new medical model include increasing the level of knowledge of medical students by placing emphasis on practical training and professional disciplines. Under the supervision of teachers, young specialists will work with patients, gaining the necessary practical experience.

Once every five years, a doctor must undergo a refresher course, which consists of attending lectures at a university for two to four months. This method acquiring knowledge is not optimal, therefore it is planned to introduce a foreign system, which includes attending trainings and master classes of leading specialists, active participation in seminars, scientific conferences etc. Thus, continuous training is carried out, the doctor constantly improves his level of knowledge, and this is the key to his successful development.

After the federal standard for third generation education came into force in the Russian Federation, the approach to training future doctors changed. What alternative has been invented to an internship?

Medicine reform

The changes were dictated by the adoption of federal laws on education. New state education standards state that graduates of medical universities must be absolutely ready to start working in the healthcare system. This is precisely what explains the need to cancel internship training. Starting from September 2017, this form of higher professional training will be completely abolished, according to the NEFU Medical Institute.

Not all students reacted unequivocally to the cancellation of the internship

Dean of the Faculty of Postgraduate Education of Doctors at NEFU MI Leonid Aprosimov notes that the concept of internship no longer exists. “Admission to internship is closed from August 30. We will graduate the last interns in August 2017 – this is a one-year specialization. This is due to the state standard focused on practical training. The need for a year's specialization after graduation is no longer required. Therefore, a medical school must train students for six years and provide them with quality knowledge. Accordingly, residency from two to five years will be considered the highest training in specialization. The duration of training depends on the specialty, labor intensity, and science intensity. All narrow specialists will undergo residency,” says the associate professor.

When studying according to new state standards, a student should already know in advance who he will be

He understands why the medical community was against such an innovation. “Everything new is met with hostility. Naturally, in the first years we will look for more negative aspects than positive ones. In fact, when studying according to the new state standards, the student should already know in advance who he will be. For this purpose, they undergo practice-oriented training,” says the candidate medical sciences. The dean of postgraduate training for doctors does not deny the fact that the internship was good because for six years the students were trained in depth in practice. Then, during the internship, they could decide who they would become and immediately try out their specialty.

Gained by losing

The chief physician of the Republican Hospital No. 2 - Center for Emergency Medical Care, Boris Andreev, is confident that it is mandatory to undergo an internship. “It was there that graduates were given knowledge, work skills, and will receive more practice. Why should yesterday's sixth-year graduate be sent straight to work? Of course, there are specialties that they can master. But it is precisely in republican large institutions, dispensaries, that people with knowledge and good practical experience“, says the candidate of medical sciences. He notes that it is during residency that students get involved in work. According to those practical skills, potential for scientific work The head of the institution can always make a choice as an employer.

Iraida Vinokurova is an ophthalmologist intern at the faculty of postgraduate training of doctors at the NEFU Medical Institute. And their graduation is the last one to complete the internship. “I think we were very lucky that we made it in time before the internship was cancelled. Next year we will receive certificates and can work in the specialty that we have chosen,” the intern shares. One of the significant advantages of canceling the internship is that after graduation and accreditation, students enter the residency immediately. After all, an internship does not provide all the opportunities that can be obtained after completing a residency. “On the one hand, for those who want to start working quickly, the internship was very good option. If we take the training of doctors as qualified specialists, then residency, of course, is better. I myself am currently undergoing an internship, but I want to go to residency next year because I want to operate. And after completing the internship, unfortunately, this is impossible,” said the future ophthalmologist.

Residency training budget expected to be cut

Resident physician at the Department of Infectious Diseases, Dermatovenereology and Physiatry at NEFU MI, Galina Krichko, notes that for some specialties, such as surgery or general therapy, internship was a “lifeline.” “A general surgeon or therapist is the door to the world of narrow specialties: neurosurgery, cardiac and vascular surgery, urology, traumatology, in therapy - gastroenterology, hematology, nephrology, cardiology, etc. Over the course of a year of training, an intern could understand what he was good at, what he liked to do and go on to specialize in residency. Now general practitioners undergo a two-year residency. What then? Another two-year residency in a specialty? Or even three or four years. And all this time the young doctor has a minimum salary. He will not be able to develop experience, plus with new system It is impossible to earn points in continuing medical education without having a job,” says the resident. In her opinion, this complicates the learning process, making studying pointlessly drawn out. But if a student decides on a specialty immediately and is accepted into residency for this specialty, this is a great success. “Saving a whole year, and with the upcoming education system even two years, is very convenient. In such specialties as anesthesiology and resuscitation, neonatology, ophthalmology, I think it is permissible to cancel the internship with the extension of residency, since people go there, as a rule, having already made up their minds. There are pros and cons, this issue is complex, and, of course, it is impossible to talk about an unambiguous solution,” said Galina Krichko.

Long-term accreditation

In 2016, the first graduation took place in the specialty “Pharmacy” and “Dentistry”. They have been studying for five years and have already begun labor activity. “To start working, they undergo specialist accreditation. This new format admission to practical activities. This the federal law. We cannot protest it,” says Leonid Aprosimov, dean of the faculty of postgraduate training of doctors at NEFU Medical Institute. When this federal law on education was adopted, the Russian Federation stipulated that starting from 2016, all doctors should switch to this format. Preparatory work for five years did not bring anything extraordinary in the medical education system. Accordingly, on December 29, 2015, another law was issued that prolongs the federal law that was called on the education of the Russian Federation, that is, the transition to specialist certification. This accreditation will be phased in over ten years.

The dean emphasizes that in the past, some students ended up in internships by accident. Everyone wants to get a narrow specialization and goes where there is budget place. And it is the current practice-oriented training that helps students go and continue their education consciously.

“The budget for residency training will be reduced. There will be big competition. But nevertheless, we do not exclude the possibility that, at the expense of their future place of work, students may remain in clinical residency, but this will be targeted training. It depends on the employers themselves.” Leonid Aprosimov, Dean of the Faculty of Postgraduate Education of Doctors, MI NEFU

Medicine has always been considered one of the most important areas social security population. In our country, representatives of this profession are treated with great respect, because everyone knows that the health of citizens largely depends on people in white coats.

In order to gain the right to personal practice, any physician has to overcome a difficult learning curve. Only after acquiring in-depth knowledge and the necessary experience can a novice doctor make a diagnosis, prescribe procedures and medications, while simultaneously taking full responsibility for the patient’s health condition.

Due to the specifics of the profession, it is customary to pay increased attention to medical education. Healthcare is one of those areas where, in addition to maintaining established traditions, it is also necessary to encourage innovation.

The need for changes in the healthcare system

Almost every day, the scientific community produces new methods for treating diseases and develops unique drug formulas in order to more effectively cope with various types of diseases.

The knowledge provided at medical universities a couple of decades ago can no longer form the necessary basis for a modern professional. The government understands this and is trying to modernize this area of education, although this is achieved with great difficulty.

Every year, officials from the relevant ministry strive to introduce fundamental changes in the educational process of aspiring doctors, now calling current system morally outdated and requiring global reform.

Another reason to think about qualitative changes in the healthcare system was the systematic complaints of citizens dissatisfied with the quality medical care and the degree of training of young specialists.

As it became known from official sources, it will bring long-awaited changes to medicine. They will affect every link in the industry, from education to reception and patient care.

The most unexpected innovations will be in the segment of postgraduate education – internship. A widespread practice, without which previously a physician could not obtain the right to practice, may very soon become a thing of history.

Reasons for canceling an internship

Beginning of reform medical field was established back in 2011, when a new federal education standard was approved at the government level. In accordance with this document, it is planned to gradually modernize the training system for domestic healthcare workers.

If previously, during the years spent at the university, students studied mostly the theory of medical sciences, and acquired practical skills during the internship, now the approach to this concept is changing radically.

According to the new standards, young people who enter a specialized university will acquire all the necessary knowledge, starting from the first year. This will be achieved through a complete restructuring of the training system, focusing primarily on practice.

Scientific and research activities will now become an integral part of the educational process. Students will carry out practical training with real patients directly in their universities or city medical institutions.

This is extremely important in terms of developing a highly qualified specialist who is able to most effectively implement the acquired knowledge in direct contact with the patient.

Everyone is like that the lesson will pass under the strict guidance of teaching staff with a high professional and pedagogical level.

Steps to cancel an internship

According to the government's plan for reforming the field of medical education, the abolition of internship will take place in three stages.

Dental and dental students will be the first to benefit from the innovation. pharmaceutical specialties: they will be relieved of the need to undergo postgraduate internship in the beginning of autumn 2016.

Pediatricians and therapists will be second in line: the reform will affect them in 2017. The final stage of canceling the internship will be 2018, and all remaining specialists will fall under it.

These changes will not affect . This practicum is often confused with an internship, although their functions are slightly different.

Residency is needed primarily for specialists with a narrow focus, such as surgeons, neurologists, dermatologists or gynecologists.

Young doctors who choose this specialization will continue to prove their professional suitability for two years and only after this period will be able to receive a special certificate allowing them to practice independently.

Advantages of changes in the system of training doctors

Such radical changes in the system of training medical specialists have received rather mixed reviews. As it turned out, people were able to find both advantages and disadvantages in such an innovation, although there were many times more positive ones.

The main advantages of the changes are:

  • reduction of specialist training time (students can start working much earlier without wasting time on additional education);
  • getting rid of the shortage of personnel in medical institutions (since medical universities will train doctors faster, the labor market will quickly fill up);
  • increasing the level of knowledge of a young doctor (by focusing on practice, the university will be able to provide more in-depth knowledge to the student during classes).

By introducing such changes, the government will be able to simultaneously speed up the production of ready-made medical personnel and increase their professional level, which is what the domestic healthcare system requires.
