Exactly at the age of 15, my first period came. Immediately irregular, with a tendency to delay (the cycle varied from 21 to 60 days, a couple of times it was up to 100 days). Since my youth I had acne, very tall height, thinness (mother is 175 cm tall and tends to be overweight, father is 190 cm thin). At the age of 16-17, she underwent examination due to mitral valve prolapse. They referred me to various doctors, including a gynecologist and an endocrinologist. According to ultrasound at that time, multifollicular ovaries; according to tests, testosterone, DHEA-S, and 17-OH were increased. The gynecologist prescribed Dexamethasone 0.5 daily, the endocrinologist agreed with the prescription. Six months later, hormones were normal with Dexamethasone, but the cycle remained irregular (from 21 to about 50 days, but without a record delay of 100 days). The face gradually cleared up, but the skin and hair remained oily (my mother, with her ideal cycle of 28 days, also has oily skin and hair). Next, you are offered the choice of either OK, or continue taking Dexamethasone. Since I was still young (17-18 years old), I continued to take Dexamethasone 0.5, 1 tablet daily for about 4 years. The cycle remained irregular, ranging from 21 to about 50 days. There were no or very few rashes, the oiliness of the skin and hair remained, did not get better or lose weight. At the age of 21, she began to be sexually active and switched to Rigevidon. Took years recently, i.e. about 7-8 years. I took 2-3 breaks from taking it for up to six months. During this period after discontinuation, the cycle was always long, up to 50 days. When taking tests after discontinuing OCs and WITHOUT taking Dexamethasone, hormone tests were normal, except for progesterone, which I took like everyone else on day 21, with a cycle of 50 days. Progesterone was reduced. I took a break in 2017: the first cycle was 44 days, ovulation on the 32nd day according to ultrasound, the second cycle - 34 days, ovulation on the 20th day according to ultrasound, the third cycle - 42 days, ovulation presumably on the 30th day according to BT measurements, the fourth cycle - I took hormones of the first phase and those that can be taken on any day - everything is within the normal range, progesterone was reduced on day 21, according to ultrasound, multifollicular ovaries, a corpus luteum cyst is in question, on day 45 I took Duphaston for 10 days, after the arrival of menstruation I started taking Regividon again. Now, after two more years of taking the OC, I stopped it again for pregnancy. And ALWAYS, when consulting with a gynecologist, if there is a delay in taking the OK, I am prescribed Duphaston and prescribed to drink it every month according to the 17-26 scheme “to start my cycle” and “due to low progesterone.” When I say that I have been diagnosed with “AGS erased form,” they shrug their shoulders. An ultrasound does not diagnose PCOS, since the ovaries are not enlarged. Question: how to diagnose 100% AGS or VDC? Is it possible that due to age, hormones have returned to normal and there is no such diagnosis? Or did this norm turn out to be an accident? Is it possible and necessary to take Duphaston or its analogues during AGS? Is it possible to donate blood for hormones during menstruation if Duphaston is discontinued? And where to go and where to start if the cycle is very long and irregular and it’s difficult to find days for the necessary examinations? (((Thanks for the answer?

Crunches in the joints

Elena, 23 years old, female

July 11, 2019 at 10:34 am

Hello, for about half a year now I have been experiencing crunches in different joints, covering all groups.

It first started after a visit to the dentist. It all started with pain in the muscles all over my body, sharp shooting pains in different parts, the neurologist said that I had intercostal neuralgia, everything went away after treatment, but the crunching continued, about 20-30 times an hour or even more, with mobility it crunches less than when I'm motionless. A week ago my leg hurt, I couldn’t walk, it seemed to give way with a sharp pain, an hour passed, I raised my leg horizontally with a bent knee, and heard crunching sounds, I raised the click and lowered the click, yesterday I checked again, everything was intact. I did exercises that didn’t help, today I got shots in my legs and arms again. What could it be? Could there be an infection? I noticed my boyfriend also had crunches, but less frequent. I think this may not be related. Could I have become infected with something at the dentist? I have a sedentary job and spend a lot of time sitting. What to do and where to go?

I drink 3-4 cups of coffee a day, I heard that coffee dehydrates, and when dehydrated, joint fluid can disappear, or maybe this has some other effect?

Alexandra, 30 years old, female

June 18, 2019 at 6:22 pm

Good afternoon After severe stress, I have been worried about dry mouth and eyes for more than a year. The dryness became less while taking antidepressants. I took tests several times over the course of a year. ESR is normal, RF is negative, AT to ENA is negative, the kit component is normal.

In June 2018, ANF is less than 1:160, in November 2018, 1:160 is a fine-granular type of luminescence, in June 2019, 1:160 is a cytoplasmic type of luminescence. On the eve of the test, I took omez and de-nol.

Strangely enough, within a year I had pyelonephritis and pneumonia. Immediately after rotavirus. Although before this I didn’t even have bronchitis or cystitis. The joints do not swell. No one in the family suffered from animmune diseases. Recently we discovered cardia insufficiency and erosion in the esophagus. Can I rule out an autoimmune process?

Hello, my 3.4-year-old son was diagnosed with vasculitis, a skin rash and swollen legs, now the obvious signs have passed, we are on bed rest, diet, the main medicine is heparin, he has been in the hospital for the second week. We suspect that the illness occurred after suffering from an acute respiratory infection.
Tell me, how do they continue to live with her? Now in life this is our diagnosis, should we be afraid of SARS so that it doesn’t happen again? Or can this be cured?
Is it possible for a child to play sports, he really loves hockey, he’s already signed up for the section, he’s been doing it for half a year, he loves training and is bored, I don’t know what to answer him, will we come back again or is sport not about us now?

All your questions can be answered only by knowing the exact final diagnosis and having information about the results of the examination and treatment, so it is better to contact your child’s attending physician or a specialist who will observe the child on an outpatient basis with all questions.

Juvenile idiopathic arthritis

Evgeniy, 33 years old, male

May 30, 2019 at 00:26

My 2-year-old daughter, a month ago, was unable to stand on her right leg in the morning. The surgeon diagnosed reactive arthritis and prescribed Nimulid. At first the pain disappeared, but on the fifth day it returned. We contacted a rheumatologist and continued taking Nimulid. After 14 days, Nimulid was replaced with diclofenac, the pain in the morning went away, but within an hour the child was limping. Ultrasound shows slight signs of improvement. Today I was diagnosed with Juvenile Rheumatoid Artiritis. Knee, ankle, hip and phalangeal joints are affected. Hospitalization and a course of methotrixate were prescribed. ROE analyzes - 3.srb -8.1, RF - 0.3. I would like to hear your opinion. Tell me what other tests need to be taken to clarify the diagnosis and determine a treatment regimen? If you can, recommend a clinic in Russia? Thank you very much in advance for your answer.

Rheumatoid arthritis can be seronegative, i.e. without characteristic markers of the inflammatory process in the blood. Considering the complaints, it is impossible to do without consulting a rheumatologist.

I was diagnosed with rheumatoid arthritis in 2016 and prescribed methotrexate. I accept. But now I don’t have any pain at all, my joints don’t bother me, is it possible to stop taking methotrexate? I’ve already tried to do this a little, but at the moment I haven’t taken it for a month and I feel fine. My doctor said that I shouldn’t interrupt. I would like to know the opinion of other specialists. Thank you.

Hello.
Methotrexate is a basic therapy drug. If you do not follow a regular schedule for taking the drug, you risk exacerbation of the disease. Stop unnecessary experiments on yourself.

Hello. I have lupus erythematosus, it doesn’t seem to be systemic, only the skin of my hands is affected. Now the disease has almost gone to sleep and has begun to reduce the dosage of medications. I'm interested in this question: will I have to take medications for the rest of my life or can I take breaks from taking them.
Is it possible to drink wine on holidays, and if so, what is the best way to proceed? Skip the dose for a few days or take the pills as usual?
I’m worried about the consequences and have given up alcohol altogether while I’m undergoing treatment and started to wonder about this issue.

Basic (main, permanent) therapy for rheumatoid arthritis consists of regularly taking medications (tablets, droppers, injections). Apitherapy and physiotherapy are auxiliary and have not proven their effectiveness, but are not the main methods of treating your diagnosis.

Hemorrhagic vasculitis and lymphadenitis

Nikolay, 47 years old, male

February 06, 2019 at 08:44 pm

Hello.

There were pinpoint hemorrhagic rashes on the skin of the lower legs and feet along the front surface, which persisted for more than two months.

General blood test and coagulogram are normal.

Other test results:

ANF less than 1:160 (negative)

ASL-o less than 50

Immunoglobulin A 4.96 (normal 0.63-4.84)

Only immunoglobulin A is abnormal

Based on this, the rheumatologist makes a diagnosis: hemorrhagic vasculitis, probably a cutaneous form. and prescribes treatment:

Take Ascorutin 1 t/3 times a day for 1 month.

Curantil 75 mg No. 40, 1 tablet 1 time per day in the evening for up to 3 months.

After ascorutin, start taking Trental 400 (400 mg) 1 tablet in the morning for 2 months.

3 weeks after the start of treatment, swelling of the gums of the lower jaw, swelling of the neck and supraclavicular muscle appeared on the left side. From the side of the swollen gums I periodically felt the taste of blood. Blood was visible in the saliva when spitting. I did an ultrasound and went to the oncology clinic and was diagnosed with submandibular lymphadenitis. During this period, a general blood test and temperature were normal.

I had an appointment with a dentist and an ENT doctor. They did not find reasons for lymphadenitis in their profile.

On the advice of a therapist, I stopped taking medications prescribed by a rheumatologist, except for ascorutin.

3 months have passed. The hemorrhagic rashes have disappeared, but moderate swelling of the neck and gums remains on the left side and

The taste of blood is occasionally felt.

I did another ultrasound.

Here are the results:

On the left, in the mandibular region, there is an oval-shaped l/u with an expanded cortical layer of 11*6 mm, with blood flow;

At the angle of the jaw, there is a hypoechoic lymph node of oval shape, with a blood flow of 21*7 mm.

On the left along the back surface of the neck there is a chain of oval-shaped hypoechoic lymph nodes,

In the lower third of the neck along the posterior surface there is a hypoechoic lymph node of an oval shape with hypervascularization, a supraclavicular lymph node on the left of normal structure 7*3 mm, oval shape.

The therapist said that the lymph nodes were enlarged, but she saw no reason for concern.

How valid is the diagnosis and treatment regimen proposed by the rheumatologist and could taking chimes cause lymphadenitis? What to do?

Consultation with a rheumatologist is necessary for deformation of ligaments, muscles and tendons, which is manifested by sharp pain, soreness, redness and swelling of the joints, limited mobility of the spine, muscle pain, deformation of the fingers and toes, as well as migrating pain in the joints. Diseases of a rheumatic nature affect not only bone tissue, joints and muscles, but also cause dysfunction of internal organs.

A consultation with a rheumatologist at ON CLINIC will help you undergo a timely diagnosis and begin treatment to restore the joy of free movement, improve your well-being and quality of life in general.

I’m 35, all my joints are cracking: my back, shoulders, and recently my knees started cracking. The crunch has been progressing for 1.5 years. Sometimes there is a slight aching pain. What to do? Where to run? I would like to receive a preliminary online consultation.

Hello! “Crunching” in the joints itself is not a disease. But if, in addition to this, joint pain begins to appear, you should consult a doctor for examination. Often such a crunch accompanies arthritis and arthrosis. We invite you to our multidisciplinary medical center for a consultation with a highly qualified rheumatologist. If necessary, a full examination can be completed with us as soon as possible. You can make an appointment with a specialist by calling 8-495-223-22-22. We will be happy to help you!

Hello, in December 2018, acute pain in the knees began after physical activity, after 3 weeks the acute pain disappeared, swelling and shooting pain remained when squatting, for example, or bending the knee at an acute angle.

In March I went to see a traumatologist, first I wrote arthrolgia, in the X-ray description they wrote gonarthrosis, the traumatologist said that the X-ray was clean, when the traumatologist referred me for procedures in the direction it was written arthrosis-arthritis.

Prescribed: meloxicam 10 injections, combilepen 2 times a week, mydocalm 150 mg tablets. Massage and magnetic laser. The swelling went away on the third day (probably from the magnet), but the pain remained when bending the knee or turning it incorrectly. 2 weeks after the course the swelling returned.

In May, I went to see a rheumatologist, did an ultrasound, the ultrasound showed bursitis, tendonitis and inflammation of the middle meniscus, the rheumatologist said reactive arthritis. Prescribed nimesil 2 rubles. a day for a week, topically and mucosat 30 injections. The second week has passed and I still can’t sit down. The swelling did not go down.

I took a complete blood test, rheumatoid factor, C-reactive protein, ALT, AST, creatinine, mycoplasma, ureaplasma, chlamydia - everything was clear.

Uric acid (UA-A) - 7.40. The traumatologist recommended that I contact a rheumatologist about diet and, after the tests improve, orthopedic insoles. Could you recommend a diet?

Good afternoon I turned to a rheumatologist, and in parallel with my complaints, he discovered that I had connective tissue dysplasia, and the doctor was able to determine this without tests, by external examination. Looking at me for the first time, he determined from me:
- joint hypermobility;
- myopia (I don’t wear glasses or contacts);
- scoliosis;
- flat feet;
- nephroptosis;
- varicose veins;
- mitral valve prolapse.

I really have all this, and the doctor said that the reason for all this + crunching in the joints is a lack of collagen in the body, and this in turn is caused by a deficiency of magnesium.

He prescribed drinking magnesium for a long time (years!) intermittently (3 months drinking, 1 month break).

I took a blood test for magnesium, and while the norm is 0.66 - 1.07, I have 0.79. Those. There is no magnesium deficiency. Does this mean that I don’t have a collagen deficiency? How can I check this?

I don’t understand why magnesium is normal, because the doctor guessed all my diseases without naming too many.

Hello! Regarding your question, you should either talk in detail with your doctor, or contact (in person) another specialist for clarification. Unfortunately, we do not have sufficient information on your condition, so we have no right to recommend anything. Hello. My legs ache and twist, in addition to trochanteric bursitis and tendonitis. Until they were discovered, 3 months passed, I passed a bunch of tests. Rheumatic factor 285, ESR 41, the rest is normal. There is no treatment, the rheumatologist only diagnosed arthrosis as stage 1. At my age (56 years old) this is almost the norm. I’m always tired, tired, I don’t sleep half the night. Please advise in which area to look for the cause. Doctors drive in circles (traumatologist to rheumatologist, neurologist and back)

Hello.
About 8 months ago the joints in my legs started to hurt. There was swelling of the fingers. All tests were normal, including culture and smear for chlamydia, except for the blood test for chlamydia, they showed the following results:
Chlamydia IgG (CP) 3.60 (normal 0.00 – 0.89)
Chlamydia IgG (titer) 1;10
Venereologists explained it as phantom and the presence of chlamydia was not diagnosed.
The rheumatologist prescribed a course of antibiotic treatment. The situation improved, the swelling subsided, the pain decreased, but the discomfort remained. Joint pain is especially acute in the morning before lunch.
A repeat blood test was taken in which the numbers still exceeded the norm:
Chlamydia IgG (CP) 2.30 (normal 0.00 – 0.89)
Please tell me, is it worth getting the next course of antibiotics, or are there any other solutions? Or perhaps the cause of joint pain is something else?

Good afternoon. I'm 27 years old, girl. Recently my joints have started cracking more often. But what worries me more is the strange quiet crunching sound (as if a lot of balloons are bursting) of the knee when you bend it. Nothing hurts. I took an x-ray. They wrote that the phenomena of arthrosis are determined in the form of sharpening of the m/condylar eminences. The ultrasound didn't find anything.

Good day.
Question for a rheumatologist. Please write your opinion.
There is a suspicion of gout or reactive arthritis.
While on vacation abroad, my temperature rose at night to the point of fever. Then on the 2nd or 3rd day the big toe joint started to hurt. Other joints also ached a little, but in the end they went away, but my leg began to hurt more and more, already on the 3rd day it was difficult to walk, I was limping severely. At this point the joint became red and swollen. I went to the hospital and they took a test for uric acid. The analysis showed 336. The norm is up to 450. They said inflammation, gave me medications and antibiotics, took it for 6 days, the pain decreased a little but did not completely go away. and even seemed to have returned. Returning to Russia, home, I went to the clinic, Terpavet prescribed a repeat test for uric acid and reactive protein. The urine test showed 306, reactive protein - 0. Doctors prescribed physio, did a magnet, laser, ultrasound. At the end I was prescribed Arcoxia and an injection of Diprospan in my finger. Now, 3 days after the injection and along with taking Arcoxia, the pain has almost gone away, so sometimes it can be a little unpleasant when you completely bend your big toe.
The question is: is it gout? Or is it just some kind of arthritis? did it become inflamed and go away? I first took a uric acid test and then 2 weeks later. Urine levels are at a good level up to 330. So I sit and suffer.

Hello! Unfortunately, it is impossible to make an accurate diagnosis in absentia. If practically nothing bothers you now, why are you suffering? To rule out gout, you can have regular blood tests to check your uric acid levels. Best regards, On Clinic.

Hello,

3 months ago I suffered from a severe flu; on about the 2nd day of the flu, pain began in both feet and swelling appeared.

After the flu, the swelling subsided, but moderate pain and morning stiffness in the foot and ankle of the right leg remained.

I haven’t seen a doctor yet, but judging by the symptoms, I suspect it’s rheumatoid arthritis.

At first I used ointments, wiped the painful area with alcohol and then applied Lyoton + Fastum gel, it became a little better but nothing more.

I decided to try mud therapy on my own (sulfide silt mud from Lake Saki), in the form of applications to the lumbar region, for 15 minutes, every other day.

I read that at the beginning of mud therapy, an aggravation occurs, my pain really worsened, moreover, my left leg, which no longer hurt, also became involved, and a slight swelling appeared in the ankle of my right leg.

The question is: is it worth continuing mud therapy procedures in this case?

It is not yet possible to see a doctor due to lack of medical care. policy. But there is no money for a private clinic yet.

P.S. I understand that if it’s arthritis, then you can’t start it either. I did not dare to self-prescribe Sulfasalazine for myself; I decided to limit myself to mud therapy for now.
